Each CTE program must complete the Arizona HOSA Statement of Assurance. This is only completed once per school year and verifies that each attendee (both student and adult) for every conference/event (including regional, state and international events) will have completed the Comprehensive Code of Conduct and Medical Liability Form.
Chapter advisors must keep the Comprehensive Code of Conduct and Medical Liability Forms for all their attendees in their possession for the duration of the conference/event, including travel to and from these conferences/events. Arizona HOSA will not collect the Comprehensive Code of Conduct and Medical Liability Forms prior to or at conferences/events.
This requirement applies to any conference, event, activity, workshop, seminar, meeting or other function sponsored or hosted by Arizona HOSA.
This Annual CTSO Compliance Form should be completed PRIOR to attending any of the above functions.
