Arizona CTSO Chapter advisers/advisors must complete this form for their individual chapters each year, by November 1, for Fall deadlines or March 1, for Spring deadlines. Each program must submit their own individual form and membership registration.
If programs collaborate with Program of Work activities or share Bylaws, Constitution, etc…, each individual program must still submit their own documents to include their specific Chapter Name. However, if two teachers teach the same program at the same school, they may submit one set of documents.
