In order to help us get you a membership invoice that includes both the national and state membership fees, we need you to complete this form letting us know what membership tier level you are requesting for this school year. FBLA Arizona will invoice you so DO NOT pay the national invoice as it does not include the state membership fees. Thank you for your help on this.

I understand that after I choose my AZ Tier Level, I will go into the National Membership System (FBLA Connect) and input my members names. I can go back and enter more throughout the year until my pre-paid membership level is reached.
